Choosing between Botox and Xeomin can feel confusing because they sound so similar. Both are injectable neuromodulators. Both soften expression lines. Both use botulinum toxin type A. Yet they are not identical, and the small differences can matter when planning cosmetic treatment.


The right choice usually comes down to the treatment area, personal response, provider preference, and medical history. Some people do beautifully with either option. Others may prefer one because of how it settles, how long it lasts, or how their body responds over time.

These are often called dynamic wrinkles. They appear or deepen when the face moves, such as when someone smiles, squints, frowns, or lifts the eyebrows. Over time, those expression lines may become visible even when the face is relaxed.


Common cosmetic concerns include:

  • Crow’s feet around the eyes

Not every use has the same regulatory status. In the United States, Botox Cosmetic has FDA approvals for moderate to severe frown lines, crow’s feet, and forehead lines in adults. Xeomin is FDA approved for temporary improvement in moderate to severe glabellar lines, the vertical frown lines between the brows, in adults.


Clinicians may use either product in other cosmetic areas when they believe it is appropriate. This is known as off-label use, and it is common in aesthetic medicine. Off-label does not automatically mean unsafe, but it does mean the provider’s training, anatomy knowledge, and judgment matter a great deal.


Their active ingredients are similar, but the formulas are different


The biggest difference starts with the formulation.


Botox contains onabotulinumtoxinA, a form of botulinum toxin type A. Xeomin contains incobotulinumtoxinA, also a form of botulinum toxin type A.


Both active ingredients come from the same general category, but the surrounding proteins differ.


Botox includes the active neurotoxin along with accessory proteins. These proteins are part of the complete formulation. Xeomin is often described as a “naked” or purified neurotoxin because it does not contain the same accessory proteins around the active molecule.


That difference is one reason some providers may suggest Xeomin for people who seem to respond less predictably to other neuromodulators. The idea is that a simpler protein profile may reduce the chance of the body forming neutralizing antibodies. True resistance to cosmetic neuromodulators is considered uncommon, but it can happen.

How both treatments work under the skin


Botox and Xeomin work by temporarily blocking nerve signals to selected muscles.


When a nerve tells a muscle to contract, it releases a chemical messenger called acetylcholine. Botulinum toxin type A interferes with that release. The treated muscle then contracts less strongly. As the muscle relaxes, the overlying skin creases less during facial movement.


The goal in cosmetic treatment is not to “freeze” the entire face. Good results usually look smoother but still natural. A skilled injector uses small, strategic doses to soften lines while preserving expression.

Botox is probably the best-known neuromodulator in the United States. Many people use “Botox” as shorthand for the whole category, even when referring to another injectable.

Many patients have heard of it and feel comfortable asking for it by name.

Botox Cosmetic is approved for frown lines, crow’s feet, and forehead lines in adults.

Because it has been used for years in cosmetic care, providers often know how it behaves in a wide range of patients.


Botox can be a strong choice for someone starting neuromodulator treatment for the first time, especially when the treatment plan includes several upper-face areas.


Xeomin has a cleaner formulation with fewer added proteins


Xeomin’s main distinction is its purified formulation. It contains the active neurotoxin without the same accessory proteins found in Botox.


Its strengths include:

This may appeal to patients who prefer a more purified formulation.


  • Potential option after reduced response


Some providers consider Xeomin when a patient feels their results from another neuromodulator are not lasting or working as expected.


  • Natural-looking softening


When dosed well, Xeomin can soften frown lines without a heavy look.

Before reconstitution, Xeomin can be stored without refrigeration, which can be useful for clinics. This does not usually affect the patient experience, but it is a practical distinction.


Xeomin is often chosen for glabellar lines, especially when someone wants a straightforward treatment for frown lines between the brows.

Results appear gradually and last for a similar amount of time


Botox and Xeomin do not work instantly. Most people begin to notice changes within a few days, with full results usually appearing within about one to two weeks.


Some patients feel Xeomin “kicks in” a little faster, while others notice no timing difference. The onset can vary based on dose, muscle strength, metabolism, treatment area, and individual response.


For both options, cosmetic results often last about three to four months. Some people metabolize neuromodulators faster and see movement return sooner. Others enjoy results a bit longer, especially after consistent treatments over time.


The duration may depend on:


  • The area treated

  • The number of units used

  • How strong the treated muscles are

  • The person’s metabolism

  • Exercise habits and lifestyle factors

  • How often treatments are repeated

  • The injector’s placement and technique


A first treatment may feel like a test run. The provider can see how the muscles respond, then refine the dose at future visits. Many people schedule maintenance treatment before full movement returns, but the right timing varies.


More product does not always mean a better result. Higher doses may last longer in some areas, but they can also increase the chance of heaviness or an overtreated look. The goal should be balanced movement, not simply maximum relaxation.


Side effects are usually mild, but risks deserve attention


Both Botox and Xeomin have similar possible side effects because they belong to the same medication class.


Common short-term effects may include:

Rare but serious risks can occur if toxin effects spread beyond the treatment area. Medical labeling for botulinum toxin products includes warnings about symptoms such as trouble swallowing, speaking, or breathing. Anyone with those symptoms after treatment should seek medical care right away.

There is no universal winner. Botox and Xeomin are both respected cosmetic injectables with strong uses. The best option depends on the face, the goal, and how the person responds.


Botox may be a better fit when someone wants a widely known option with multiple cosmetic approvals and a long track record across forehead lines, crow’s feet, and frown lines. It is also a familiar choice for people who have used it before and liked the result.


Xeomin may be a better fit when someone wants a purified formulation without accessory proteins, especially for glabellar lines. It may also be considered when past results with another neuromodulator seemed weaker than expected, though the cause should be evaluated carefully.


A good consultation should cover:


  • Which areas bother the patient most

  • Whether the lines are dynamic, static, or both

  • Past injectable history

  • Desired level of movement

  • Medical history and contraindications

  • Expected onset and duration

  • Possible side effects
